Your First Visit
Welcome to Windermere Dentistry! Our whole staff can’t wait to welcome you in and help you feel right at home from the very first appointment. We have two wonderful staff members at our front desk that will greet you warmly and ensure that all of your paperwork is completed. Then, you’ll be courteously brought back to the treatment room and walked through a comprehensive examination with one of our doctors, who will listen to your concerns and provide quality solutions – no rushing! Our goal is to leave you feeling great about your visit and eager to schedule another one.
New Patient Forms
For your convenience, we’ve made the necessary new patient forms available here online. Just follow the links below to download the files, print them out, and complete the information in the comfort of your own home. Just make sure to remember to bring them with you to your first appointment! If you’re unable to properly print the paperwork, please let our office know – we can mail or fax them.
Financing Available
If you don’t currently have dental insurance (or if your insurance doesn’t provide adequate coverage for your chosen treatment plan, such as a cosmetic makeover), there is another option. CareCredit offers affordable monthly payment plans that come with little (or even NO) interest attached.
